Clients:  Western Maine Family Health Center (WMFHC) is a non-profit 501(c) organization located in Livermore Falls, bordering the Androscoggin River. The Health Center is twenty minutes south of Farmington and is surrounded by rural communities. WMFHC is one of the eleven practices of HealthReach Community Health Centers, which delivers medical care in central and western Maine to 36,000 patients each year.
Services:  In 1987, the HealthReach family welcomed Western Maine Family Health Center. In 2010 the practice served 2,690 patients. The variety of services we provide ensures that all patients receive accessible quality care. We offer primary care and preventive services including routine physicals, chronic disease management, acute visits, minor surgery, and referrals.

Our providers collaborate with a team of physicians at Franklin Memorial Hospital in Farmington, where hospital care for our patients is provided. We also work with other state and community partners to provide on-site access to programs including Breast and Cervical Health, Employee Assistance, Nicotine Replacement Therapy, and Raising Readers.

Our health center and our staff are committed to the community, playing a vital role in supporting and aiding our neighbors through quality health care. The towns served by the health center include Canton, Fayette, Hartford, Jay, Livermore, Livermore Falls, and Sumner. In addition, patients from surrounding towns such as Chesterville, Farmington, New Sharon, Wilton and others are regularly welcomed.

The Androscoggin River, once busy with log drives, flows through Livermore Falls. Memories of the railroad and paper industry pervade the community, creating a historically compelling area. Only 12 miles south of Farmington, and 25 miles northwest of Augusta, this rural community offers year-round activities for the whole family. Snowmobiling, boating, and gorgeous views entice travelers and residents alike.

Fees: Payment is expected at time of service. If you cannot pay at this time, please make arrangements with the office before being seen. Personal checks, cash, Visa and MasterCard are accepted. Medicare, MaineCare, Anthem and most other insurance companies will automatically be billed. With many insurances, a co-pay is due at the time of your visit.
An affordable Care Program is available to help patients who have trouble meeting the cost of health services. This plan is based on income and family size. Applications are available in each of the exam rooms, or by contacting the receptionist. All applications are confidential.

Founded in 1989, Healthy Community Coalition (HCC) is an affiliate of the Franklin Community Health Network with a mission of promoting health and well-being in Franklin County and neighboring towns, including Livermore Falls.

The Franklin Resource Network

The mission of the Franklin Resource Network is to maximize resources in our community, enabling elders, people with disabilities, caregivers, and all residents to easily find information about, and gain access to, services and opportunities to contribute their gifts and talents.

This project is funded, in part, by an Aging and Disability Resource Center grant from the federal Administration on Aging and Center for Medicare and Medicaid services.
