Clients:  Individuals interested in disability resources.
Services:  Services provided by state agencies are usually organized by funding sources rather than by consumers' needs. To get the services that you, a family member or a client needs, you may have to talk to many different departments or bureaus within the state system. To complicate matters, some of the program names can be very similar, but they may provide different services. Sometimes departments provide services to you directly; more often they contract to community-based providers.

In this document we have listed the state agencies that consumers with disabilities are likely to contact. The information for each department or office includes the following:

* a brief description of the department or office;
* the people they serve;
* the names of the programs or offices they administer;
* the types of programs and services that they provide or pay for; and
* contact Information.

Use the Site Map or the “Site Search” engine to find what you need. We’ve gathered together a collection of many useful links, short “tip sheets” that offer helpful hints, and toolkits on various topics. Toolkits are resource guides that allow you to pick and choose the information you need. Tip Sheets are short, helpful hints on lots of different topics. Healthy Community Coalition

Founded in 1989, Healthy Community Coalition (HCC) is an affiliate of the Franklin Community Health Network with a mission of promoting health and well-being in Franklin County and neighboring towns, including Livermore Falls.

The Franklin Resource Network

The mission of the Franklin Resource Network is to maximize resources in our community, enabling elders, people with disabilities, caregivers, and all residents to easily find information about, and gain access to, services and opportunities to contribute their gifts and talents.

This project is funded, in part, by an Aging and Disability Resource Center grant from the federal Administration on Aging and Center for Medicare and Medicaid services.
