Services:  Persons with Alzheimer’s disease or other dementia, their families, and the community. Residential care facility with 30 beds, overnight respite, adult day services, support groups, caregiver training/educational services, consulting services, including environment surveys, building and program design, associated with Geriatric Evaluation unit. We hope to enhance and sustain the quality of life for individuals with Alzheimer’s, related dementia, and families and loved ones involved with their lives.

Clients:  18 and older.
Services:  The State of Maine’s Community Assessment Program provides health care assessments to individuals over 18 years of age, through a referral from a medical service provider. In 1998 GHS took over management of the program, when it made the transition from paper to electronic assessments. The GHS Community Assessment Program team is comprised of more than 60 nurse assessors, and supported by administrative staff who coordinate the assessments from hospitals, nursing facilities, individuals’ homes, and other health care facilities. We also staff a toll-free help desk to receive referrals from medical providers and answer questions about existing or past cases. Each of our nurse assessors carries a laptop computer. After each assessment, the information collected is directly relayed to our office and to the Bureau of Elder and Adult Services, via a statewide network of secure dial-in locations.

Clients:  Consumers with a disability who want to maintain their level or reach a higher level of independence.
Services:  Independent Living Centers are typically non-residential, private, non-profit, consumer-controlled, community-based organizations providing services and advocacy by and for persons with all types of disabilities. Their goal is to assist individuals with disabilities to achieve their maximum potential within their families and communities.
Fees:  No cost to consumer.
Hours:  Central office: Monday thru Friday, 8:00 a.m. – 4:30 p.m. Answering machine on 24-hours a day.

Clients:  Individuals who need to move within the continuum from assisted living and residential care, utilizing their highest level of independence, to adult day services and long term care for those needing more assistance. Assisted living enables older adults to live in independent apartments while enjoying congregate services, such as meals, housekeeping, transportation and activities. Adult day services can provide needed relief for caregivers throughout the day and allows a smooth transition to residential care as more supervision is needed.

Post acute care is available for Medicare short-term skilled nursing needs between hospital and home. This focus centers on post acute rehabilitation utilizing professional physical, occupational and speech language pathologists for strengthening and endurance. Rehabilitation can be extended once the patient returns home through our on-site outpatient rehabilitation programs. Skilled nursing care provides patient monitoring, wound care and personal assistance in managing health issues. Palliative care is provided with particular emphasis on comfort and support for the families and individuals approaching end stages of life.

With long term care and skilled nursing facilities in seventeen locations in Maine and Massachusetts, the company provides specialty services ranging from post acute and Medicare skilled nursing care as well as rehabilitation for patients who have follow up needs from a hospital stay. North Country also offers the older adult a range of residential care services focusing on the mobile individual in short or long term care situations. Assisted living apartments and adult day services complement the variety offered by North Country.
