The American Cancer Society provides support for women dealing with the emotional and physical effects of breast cancer. Volunteers visit breast cancer patients, answer questions and share personal experiences. Early support is also available for women who have detected a breast lump. Reach To Recovery volunteers are positive role models. Man To man The Man to Man program helps men cope with prostate cancer by offering community-based education and support for patients and their family members. A core component of the program is the self-help and/or support group. Volunteers organize these free monthly meetings where speakers and participants learn about and discuss information about prostate cancer, treatment, side effects, and how to cope with prostate cancer and its treatment. Some Man to Man groups may invite wives and partners to attend meetings. In other locations, wives and partners may meet separately in a group setting called Side by Side |

| Services: | Skilled Home Care- Skilled home care services assist both children and adults who need short term care in their recovery from illness, injury, surgery or help with a changing medical condition. Weekly home visits by your nurse or therapist are coordinated under a care plan approved by a physician. Along with caregiver support from family and friends, services from AHCH social worker, home health aide or visiting volunteers are also provided when needed. Coverage provided through Medicare, MaineCare, and health insurance plans. Subsidized care is also available based on need. Outpatient Mental Health Services- AHCH now offers children and adults with severe chronic and disabling mental health conditions the option of receiving therapy and counseling services in their homes or other settings, such as schools, shelters, and adult and child day care centers. Eligibility for services limited to clients with MaineCare coverage. Hospice- Hospice offers a special program of comfort care for individuals of all ages nearing the end of life and bereavement support for families and caregivers. The choice of hospice care is made in consultation with your physician when your medical condition and prognosis no longer makes curative treatment viable or desirable. Whether you live at home or in a care facility, hospice brings quality to the time spent with family and friends by addressing your physical and emotional comfort through pain and symptom management, counseling and spiritual and volunteer support. Coverage provided through Medicare, MaineCare, and health insurance plans. Subsidized care is available based on need. Supportive care- Supportive care provides individuals and families with extra help in maintaining safety and independence at home with affordable care services. Services range from help with household tasks and personal care to companionship, respite for family caregivers and specialized nursing. You can flexibly purchase services for a few hours a week or several times a month-up to 24 hours a day. Unlike skilled home care, you directly manage most supportive care services without pre-approval of a physician. Private pay with coverage through long-term care insurance and some health insurance plans. Lifeline- Residents in the Oxford, Franklin, and northern Cumberland Counties can purchase Lifeline Personal Response Services through the AHCH. The wearable Lifeline device provides a quick telephone response from trained Lifeline staff in the event of a medical emergency. Clients pay a one-time installment fee and a monthly service charge. |

About our therapists: Melissa Catlin, LCSW provides adolescent, adult individual and couples’ outpatient therapy. Melissa has over 10 years of experience in the outpatient setting and specializes in work with trauma, anxiety and depression and facilitates Dialectical Behavioral Therapy (DBT) groups. Corey Walmer, LCSW works with all ages with an emphasis on child, adolescent and family needs. | Services: | The mission of Franklin Health Access is to improve the health and well-being of the uninsured and underinsured residents of Greater Franklin County through advocacy, education, treatment, and access to services. Franklin Health Access welcomes people of all ages that have no health insurance or insurance with a deductible of $5000 or greater and that meet the eligibility criteria.
If you do not have a doctor, we will help you choose one that participates in this program. FHA will assign a Personal Health Advocate who will help with healthcare needs and also refer you to other available community resources. Services are based on a sliding-fee scale, and free self-help information is provided. FHA offers a free ScoreHealth screening for cholesterol, blood pressure, and heart risk factors, and for prescription reimbursements of 50% of your cost up to $100 (maximum). Professional cosmetologists and hair stylists offer advice and tips for coping with hair loss and skin changes. Volunteer beauty professionals lead small groups, usually consisting of 6 to 10 women, through practical, hands-on experience. Women learn about makeup techniques, skincare, nail care, and options related to hair loss such as wigs, turbans, and scarves. Each group program participant receives a free kit of cosmetics for use during and after the workshop. For patients who are unable to attend a group workshop, a free, one-time, individual salon consultation with a volunteer cosmetologist may be available in their area. These trained beauty experts help each patient address her specific skin, hair, and related appearance needs. |

| Services: | One in 9 people are affected by kidney disease. In Maine that is more than 100,000 people. The NKFM organization provides direct financial assistance to people on dialysis. It also offers preventative FREE Kidney Early Evaluation Program (KEEP) health screenings across the State of Maine. To educate it offers a program called Kidneys in the Classroom (KIC) to kids in grades 3-5. KIC outlines where an individual's kidneys are, what they do, and living a healthy lifestyle to keep the kidneys healthy. The NKFM also distributes brochures to people interested in learning more about kidney disease.
The National Kidney Foundation of Maine has been working to grow within the communities of Maine. Over the past 2 years The NKFM has distributed over $110,000 in direct financial assistance to patients for food, medication, transportation, and utilities. |

| Clients: | Strong Area Health Center (SAHC) has been part of the Franklin County community since 1986. Many of the staff live locally and have deep family roots in the area. Avon, New Vineyard, Phillips and Strong are the official towns served by the health center, but patients from Farmington and other towns utilize the health center services as well. The Health Center staff and community members worked together to plan a more modern building and in July 2001, that dream was realized when the Health Center moved into a new, spacious home.
In January 2003, another expansion was completed: a state-of-the-art Dental Center. Strong was selected as the first HealthReach facility to offer dental care. Recent studies have demonstrated a greater link between oral health and overall health than was previously realized. A coalition of local residents and service providers worked with HealthReach for several years to plan the program and seek funding for this important endeavor. | Fees: | Cost depends on service requested. All major insurance companies and managed care health plan, including Aetna, Anthem Blue Cross and Cigna, as well as Medicare and Medicaid are accepted.
A Sliding Fee Program is offered through funding from the U.S. Bureau of Primary health Care. This program, based on family size and income, is available for patients who qualify. |
