| Services: | Skilled Home Care- Skilled home care services assist both children and adults who need short term care in their recovery from illness, injury, surgery or help with a changing medical condition. Weekly home visits by your nurse or therapist are coordinated under a care plan approved by a physician. Along with caregiver support from family and friends, services from AHCH social worker, home health aide or visiting volunteers are also provided when needed. Coverage provided through Medicare, MaineCare, and health insurance plans. Subsidized care is also available based on need. Outpatient Mental Health Services- AHCH now offers children and adults with severe chronic and disabling mental health conditions the option of receiving therapy and counseling services in their homes or other settings, such as schools, shelters, and adult and child day care centers. Eligibility for services limited to clients with MaineCare coverage. Hospice- Hospice offers a special program of comfort care for individuals of all ages nearing the end of life and bereavement support for families and caregivers. The choice of hospice care is made in consultation with your physician when your medical condition and prognosis no longer makes curative treatment viable or desirable. Whether you live at home or in a care facility, hospice brings quality to the time spent with family and friends by addressing your physical and emotional comfort through pain and symptom management, counseling and spiritual and volunteer support. Coverage provided through Medicare, MaineCare, and health insurance plans. Subsidized care is available based on need. Supportive care- Supportive care provides individuals and families with extra help in maintaining safety and independence at home with affordable care services. Services range from help with household tasks and personal care to companionship, respite for family caregivers and specialized nursing. You can flexibly purchase services for a few hours a week or several times a month-up to 24 hours a day. Unlike skilled home care, you directly manage most supportive care services without pre-approval of a physician. Private pay with coverage through long-term care insurance and some health insurance plans. Lifeline- Residents in the Oxford, Franklin, and northern Cumberland Counties can purchase Lifeline Personal Response Services through the AHCH. The wearable Lifeline device provides a quick telephone response from trained Lifeline staff in the event of a medical emergency. Clients pay a one-time installment fee and a monthly service charge. |

| Services: | Annie’s Beanpole is a project to loan medical equipment, free of charge and with no due dates, to those who cannot afford it. To date, the project has loaned out over thirty pieces of medical equipment and assistive devices to almost twenty individuals and their families. These include wheelchairs, shower seats, toilet extenders, bedside commodes, bed rails, OT devices, walkers, crutches, nebulizers and supplies, and exercise equipment, including a stationary bike. Anyone who has equipment in good condition can find a home for it here, where it will be put to good use. Annie’s Beanpole is also now accepting cash donations to fund the purchase of frequently requested items. Please note, however, that your donations are NOT tax deductible. Outpatient mental health services for children, adolescents, adults and families at our Wilton facility located in the historic Bass Building in downtown Wilton. In Home Support services (65M program) is available for children up to age 21 with a major mental health diagnosis who may require more extensive services. The program, available to MaineCare residents, provides individualized services in the home setting for youth and their families. Please call the office @ 645-5304 to initiate referral. Habilitation program (Section 24). This program serves children up to age 21 with a diagnosis of Mental Retardation or Autism Spectrum Disorders and assists them in activities of daily living in order that they might remain in their homes. This service, available for high needs children and adolescents with a mental health diagnosis, provides families with targeted support and access to area resources. This program is available for MaineCare residents. There is no additional charge to clients for this service. Home Health Services are provided in home to assist clients with activities of daily living and nursing services. Home health care is staffed by Registered Nurses ( RNs), Licensed Practice Nurses (LPNs), Certified Nursing Assistants (CNAs) and Personal Support Specialists (PSS). Please call the office @ 645-5304 to initiate referral. Home Modification Specialists of Maine offers services to help clients maintain more independence and meet the needs of family members. Available services include installation of adaptive aids such as ramps, grab bars etc., repairs, remodeling, painting, yard work and more. About our therapists: Melissa Catlin, LCSW provides adolescent, adult individual and couples’ outpatient therapy. Melissa has over 10 years of experience in the outpatient setting and specializes in work with trauma, anxiety and depression and facilitates Dialectical Behavioral Therapy (DBT) groups. Corey Walmer, LCSW works with all ages with an emphasis on child, adolescent and family needs. She utilizes a variety of approaches including play therapy, cognitive behavioral as well as narrative therapy. She also facilitates Dialectical Behavioral Therapy groups (DBT). She is a member of the National Association for Play Therapy. Stuart Beddie, LCPC, NCC has over 12 years experience in clinical work with children, adolescents and families dealing with severe emotional and behavioral issues. He specializes in treatment of sexual trauma and sexually aggressive behaviors. Matthew Deming, LCSW works with all ages and the full spectrum of individual (child, adolescent and adult) and family needs. In addition to carrying an outpatient caseload, Matthew is the Clinical Team Leader for the Waterville and Wilton offices. Marilyn McKay, LCSW has 28 years of experience as a therapist. She works with all ages. Areas of special interest include adolescents, hypnotherapy, women’s issues, death and dying, AIDS and Gay/Lesbian issues. Steve Brod, LCSW works with individuals of all ages as well as families and couples. Steve has advanced training in family therapy, crisis stabilization, sexual dysfunction, supervision, trauma debriefing and Autism Spectrum Disorders.

| Services: | Elder Independence of Maine connects over 3,000 consumers throughout Maine with home care services. EIM coordinates the delivery of in-home services through a network of 200 contracted providers. EIM is determined to service all consumers with a care management system that: Equips consumers with information about care options. Protects the consumers’ right to self-direct. Coordinates service in a manner that seeks to reduce fragmentation of care, promote quality of life and positively influence social welfare. Advocates on behalf of the consumer. | Services: | The State of Maine’s Community Assessment Program provides health care assessments to individuals over 18 years of age, through a referral from a medical service provider. In 1998 GHS took over management of the program, when it made the transition from paper to electronic assessments. The GHS Community Assessment Program team is comprised of more than 60 nurse assessors, and supported by administrative staff who coordinate the assessments from hospitals, nursing facilities, individuals’ homes, and other health care facilities. We also staff a toll-free help desk to receive referrals from medical providers and answer questions about existing or past cases. Each of our nurse assessors carries a laptop computer. After each assessment, the information collected is directly relayed to our office and to the Bureau of Elder and Adult Services, via a statewide network of secure dial-in locations. |

| Services: | The Office of Elder Services promotes programs and services for older adults, their families and for people with disabilities. These programs and services are:
Adult Protective Services Investigates allegations of abuse, neglect or exploitation of adults age 18 and older. Provides protective services, petitions Probate Court to become public guardian or conservator for incapacitated individuals who cannot direct their own affairs. Manages assets of public wards and develops specialized housing and other services. Coordinates Department of Human Services activities under the AMHI Consent Decree. Trains other health care, social service and law enforcement agencies. Programs serves 3,600 people annually, including 603 in public guardianship. Adult Services staff adhere to the Code of Ethics of the National Association of Social Workers Home and Community Based Care: Manages programs involving home health and other supportive services to 4,100 elderly and disabled adults in order to avoid or delay nursing home placement. Programs include: Home Based Care; Congregate Housing Services; Alzheimer's Respite; Adult Day Care; Medicaid Waivers for Elderly, Adults with Disabilities and Physically Disabled Adults; Homemaker Services and the Long-term Care Ombudsman Program. Manages statewide pre- admissions assessment of all nursing facility applicants through a contract with Goold Health Systems. Community Options Unit helps consumers and their families' problems. Resource Development: Responsible for developing publicly assisted residential care facilities, assisted living facilities and adult family care homes. Administers funds for adult day services and contracts for affordable congregate housing services programs. Conducts mandated reviews under the Maine Certificate of Need Act for nursing facility projects, such as acquisitions, construction and renovation. Issues "requests for proposals" for the development of facilities for special populations, such as persons with Alzheimer's disease, acquired brain injury, or seriously mentally ill public wards, as funds become available for expanding programs. Regulates/licenses assisted living programs, residential care facilities, adult family care homes, and adult day care programs [residential care facilities that are part of a licensed nursing facility are regulated by the Bureau of Medical Services, Division of Licensing and Certification]. Community Services: Supported primarily with federal Older Americans Act funds, provides home delivered meals, outreach, information and assistance, public transit, employment, volunteer, public education and legal services to 42,000 people annually through the five Area Agencies on Aging and Legal Services for the Elderly, Inc. Also includes federal demonstration grants for Alzheimer's services and health insurance counseling. Disability Determination Services: 100% federally funded, the DDS staff of 60 employees and 13 contract physicians and psychologists make 22,000 new decisions for Social Security Disability and Supplemental Security Income claims affecting 17,000 people annually. |
